A mistake log is not a list of everything you got wrong. If every entry says “forgot this” or names an entire topic, the log becomes a second specification that you never want to open. Its job is to catch patterns that a total mark hides. Two students can both lose ten marks on a paper while needing completely different revision: one may lack core knowledge, while the other knows the content but misreads instructions and rushes calculations.
The best log is brief enough to complete while marking and specific enough to change your next attempt. Do not copy the question or mark scheme. Record the official paper reference, your own summary of the task, why the response failed, the correction in your own words and when you will test it again.
Record seven fields and stop there
A compact entry should tell your future self what happened without recreating the full paper. Record the date, qualification and paper reference, subject or topic, your own short description of the task, marks lost, cause, corrected method and retry date. Do not paste the question, source text, diagram, mark scheme or examiner wording into a public document. Link back to the board's official paper instead. A summary such as “interpreting a rate graph and explaining the plateau” is enough. The effort of naming the tested skill is also part of the diagnosis. If you cannot describe what the question was testing, check the specification or ask your teacher.

Classify why the mark was lost
The topic tells you where the mistake appeared. The cause tells you what to do. Use a small set of categories consistently: knowledge, prerequisite, interpretation, command word, method, evidence or application, calculation, timing and checking. You can add a subject-specific label if it leads to a different task, but avoid creating twenty categories that you cannot review.
Choose the earliest cause in the chain. Suppose a chemistry answer uses the wrong equation, produces the wrong value and then gives the wrong unit. If you did not know which equation represented the process, the main cause is knowledge or application. If the equation was correct and only the conversion failed, it is calculation. If every step was correct but the unit was omitted, it is checking. One entry can carry a secondary tag, but the first repair should target the root.

Write a correction you could use under pressure
A correction should be an instruction, not a copied answer. “Read the question” is too vague. “Circle the required variable and write its unit before choosing an equation” is observable. “Add more analysis” is equally weak. “After each quotation, name the precise word or structural choice and explain how it supports my argument” gives you a move you can rehearse.
For factual gaps, write the smallest accurate knowledge unit and where it fits. For methods, write the sequence. For extended responses, identify which assessment objective was missing and describe a move that would provide evidence for it. Check the exact board materials because a high-quality answer in one subject may require different combinations of knowledge, application, analysis and evaluation from another.

Use an immediate repair and a delayed retry
First, repair the attempt while feedback is available. Close the mark scheme, cover the model and redo the missed step from a blank starting point. Explain why the new approach earns credit. This checks that you understood the feedback, but it does not prove that you can retrieve it later.
Then set a delayed retry using a different but closely matched question. A short gap of a few days is practical for many revision schedules, followed by another check later if the error is important or repeated. These are planning intervals rather than magic numbers. Adjust them around your course and exam date. Mark the entry secure only when you can identify the method, complete the task and explain the key decision without reopening the correction.

Make the log fit the subject
In maths and numerical science, capture the first incorrect decision rather than only the final answer. Useful method tags include selecting a formula, rearranging, sign handling, units, significant figures and interpreting the result. Official schemes may award method, accuracy or follow-through marks, so read the notation and instructions for your board before deciding what was actually lost.
In essay subjects, a mark is often awarded through a level or band that considers the response as a whole. Do not invent a precise claim that one sentence lost two marks. Compare the response with each assessment objective and record the weakest feature you can demonstrate, such as unsupported judgement, generic context or undeveloped comparison. In languages, separate communication, task fulfilment, vocabulary, grammar and receptive misunderstanding according to the qualification's published grid.
Review patterns once a week
A log becomes valuable when entries can be grouped. Once a week, count recurring causes and scan the topics attached to them. If command-word errors appear across biology, geography and psychology, practise unpacking the task in several subjects. If algebra blocks chemistry and physics calculations, the shared prerequisite deserves priority. If most entries are isolated facts from different topics, short retrieval practice may be more useful than another full paper.
Look at whether retries are improving, not just whether the list is growing. A large log can mean you are doing ambitious practice, which is not a problem. Archive secure entries so the active view stays manageable. If the same category remains dominant after several genuine retries, change the practice rather than copying the correction again.
Keep the log useful, not punishing
Do not log a vague “careless mistake” and assume the problem is your personality. Name the behaviour: copied a value incorrectly, skipped the unit, answered the previous part, changed a sign or did not return to a flagged question. Each has a different prevention step. If tiredness, stress or inaccessible conditions are repeatedly affecting attempts, the right response may involve rest, a conversation with school or using agreed access arrangements, not a harsher tracker.

Should I record every mistake?
No. Correct a one-off slip on the paper, but log mistakes that reveal missing knowledge, a weak method, a repeated habit or a high-value prerequisite. Logging everything makes the important patterns harder to see.
What should I write if I do not know why I lost the mark?
Mark the cause as uncertain and compare the response with the official scheme, specification and any exemplar. Ask your teacher a narrow question about the first point where your response stopped meeting the criteria. Do not guess a detailed cause from the final score.
How often should I retry a mistake?
Retry until the underlying skill works independently on a fresh question. An immediate repair plus a later attempt is a useful minimum, but important or recurring skills may need another check after a longer gap.
Can I paste past-paper questions into my tracker?
Keep the board's material on its official website. Store the paper reference, question number and your own short description instead. This is enough to find the question again and avoids reproducing copyrighted exam material.
