Seeing the same error on another paper is frustrating because you have already read the correction. The problem is that recognising a correct answer and producing one independently are different tasks. A mark scheme makes the missing step visible. Unless you later have to choose and use that step without the scheme, the correction can remain familiar but unavailable.
Repeated mistakes also get labelled too broadly. “Careless” might mean copying a number, losing a negative sign, overlooking a limit in the question or failing to leave time for a check. Those are different behaviours with different fixes. The aim is not to eliminate every human slip. It is to make important errors less frequent, easier to catch and less costly.
Recognising a correction creates an illusion of security
When the mark scheme is open, the correct route looks clear. If you immediately redo the question, you may remember its layout, values or wording. That confirms that you understood the feedback in the moment, which is useful, but it does not show that you can retrieve the approach later or recognise it in a different context.
Use two checks. First, close the feedback and repair the original response from a blank starting point. Second, after a gap, attempt a fresh question with different surface details and the same underlying decision. If the method only works when the question looks familiar, it is still tied to that example rather than ready for the exam.
Replace “careless” with an observable cause
Calling an error careless feels explanatory, but it does not produce a practice task. Describe the point where the response changed: copied 0.06 as 0.6, used the previous part's value, answered about the wrong character, gave two separate descriptions instead of a comparison or spent too long perfecting an early answer.
Then choose a prevention step that happens at the same point. For copied values, touch or tick each figure as it enters the working. For negative signs, carry brackets through substitution and run a sign check. For drifting from an essay question, repeat a short version of the task at the top of each paragraph plan. For timing, use a checkpoint and decide in advance what moving on looks like.

Find the earliest break in the process
A repeated wrong answer may be the final symptom of an earlier gap. Use a simple chain: knowledge, recognition, method, communication, checking. Did you know the relevant idea? Did you recognise that it applied? Could you carry out the steps? Did you present them in the form the question rewarded? Could a planned check have detected the problem?
Suppose you repeatedly lose marks on chemistry mole calculations. If the algebra fails when rearranging, more chemistry notes will not repair the prerequisite. If the algebra works once the equation is supplied, practise selecting the relationship from the quantities and units. If both are secure but you omit units, build a final-line check. The earliest weak link determines the first task.
Build a method you can actually run
Advice such as “check your work” or “add evaluation” is too vague under time pressure. Turn it into a short sequence with a trigger. The trigger is what you notice in the question; the sequence is what you do next. For a percentage-change task, the trigger may be two values at different times. The sequence may be difference, divide by original, multiply by one hundred, then sense-check the direction.
For an extended response, the trigger might be a judgement command. The method could be: define the criterion, develop two relevant lines of reasoning, test each against context or evidence, then state which carries more weight and why. This is not a universal essay template. Adapt it to the published assessment objectives and examples for that subject.

Retry after a gap and add variation
An immediate retry checks understanding. A delayed retry checks retrieval. A varied retry checks whether the learning transfers. Use all three for recurring, high-value errors. The gaps do not need a complicated algorithm: pick a practical date a few days later, then revisit important skills again after a longer interval if they remain relevant.
Variation should preserve the underlying decision while changing the surface. Change the diagram but keep the same geometric relationship. Change the biological context but keep the same causal process. Change the text extract while keeping the analytical demand. Change the economic case while keeping the chain of reasoning. If performance collapses, the correction may have been memorised too narrowly.
Design checks for the errors you actually make
A final instruction to “check everything” is unrealistic. Build a small check from your own patterns. In maths or physics, estimate the sign, size and unit of the final answer. In biology or chemistry, check that technical terms describe the correct process and that the explanation reaches the observed outcome. In English, check that every paragraph advances the set question rather than a memorised theme.
Run checks at useful moments, not only in the last minute. Check a converted value before it feeds several calculations. Reread an essay's working judgement before planning the next section. Confirm that paired evidence truly compares both items. Practise the check during revision until it becomes part of the method rather than an extra promise.
Change the practice when the pattern does not change
If the same error survives several honest retries, doing more identical questions is not automatically the answer. Revisit the diagnosis. You may be missing an earlier concept, using an explanation you cannot reconstruct, practising with too much scaffolding or marking yourself too generously. Ask your teacher a narrow question and bring the original attempt, your diagnosis and the later retry.
You can also reduce the task temporarily. Practise selecting a method before solving, planning claims before writing paragraphs or identifying variables before analysing a practical. Know when another kind of support is needed
Not every repeated difficulty is solved by a stricter revision system. If the paper format is inaccessible, check that you are practising with the access arrangements and modified materials formally agreed through your school or college. If instructions or content remain unclear, ask your teacher rather than repeatedly self-marking an uncertain interpretation.

Why do I repeat a mistake even after understanding the mark scheme?
